Now that you've printed a copy of the Intake Questionnaire,  you'll want to begin by filling out the demographic information on Page 1 and continue through each of the ten pages, ignoring information that isn't relevant to you.

I can't emphasize enough how important it is for this information to be as complete as you can make it, and that I have time to review it before, or on the day of, our first meeting.  If you're able to email the completed form, please send it to stirredh2o@gmail.com.  Otherwise, you can snail-mail it to the store address below, or bring it with you to your first appointment.
